Cardiac Tone in Sleep Related Alveolar Hypoventilation

چکیده
933 Heart Rate Variability in Sleep Related Alveolar Hypoventilation—Palma et al INTRODUCTION Sleep related alveolar hypoventilation (SRAH) is characterized by decreased alveolar ventilation resulting in sleep related oxygen arterial desaturation in patients with normal mechanical properties of the lung. SRAH can be distinguished from central and obstructive sleep apnea (OSA) by the fact that OSA is defined by periodic alterations in the respiratory flow (i.e., apnea, hypopnea, or respiratory effort-related arousals [RERAs]) that are not typically present in SRAH, even though both conditions (sleep apnea and SRAH) may coexist.1 In the absence of pulmonary parenchymal or vascular pathology, lower airway obstruction, neuromuscular disorders, or kyphoscoliosis, the disorder is referred to as idiopathic SRAH. When severe obesity is present, it predisposes individuals to developing SRAH by increasing the burden on disadvantaged respiratory muscles, and then the disorder can be referred to as obesity-related sleep hypoventilation syndrome, and is usually included in the spectrum of obesity-related hypoventilation CARDIAC TONE IN SLEEP RELATED ALVEOLAR HYPOVENTILATION

Recurrence of sleep apnea syndrome following tracheostomy. A shift from obstructive to central apnea.
This report describes an unusual case of severe obstructive sleep apnea and alveolar hypoventilation leading to hypersomnolence and cor pulmonale, which were corrected by tracheostomy. Four years later, after a 22.5-kg weight gain, nocturnal apneas of similar frequency, duration, and depth of desaturation reappeared but were totally central in origin. The central apneas were eliminated with hom...
متن کاملCoexistence of obstructive sleep apnea and chronic obstructive pulmonary disease
Background: Obstructive sleep apnea (OSA) and chronic obstructive pulmonary disease (COPD) may lead to chronic alveolar hypoventilation. The coexistence of OSA and COPD has been termed the ‘overlap syndrome’. The aim of the study was to determine the relationship between the severity of COPD and the occurrence of chronic alveolar hypoventilation in patients with OSA and to evaluate the impact o...
متن کاملROHHAD Syndrome: Reasons for Diagnostic Difficulties in Obesity
A very rare syndrome of rapid-onset obesity with hypoventilation, hypothalamic dysfunction and autonomic dysregulation (ROHHAD) has been recently described as causing morbidity due to hypothalamic dysfunction and respiratory arrest. Its prognosis is poor and often cardiac arrest occurs due to alveolar hypoventilation. This disorder can mimic genetic obesity syndromes and several endocrine disor...
متن کاملCentral alveolar hypoventilation and sleep. Treatment by intermittent positive-pressure ventilation through nasal mask in an adult.
Idiopathic central alveolar hypoventilation, uncommonly seen in middle-aged adults, has often been treated by tracheostomy and assisted ventilation during sleep or by implantation of a diaphragmatic pacemaker with or without tracheostomy. We report the successful treatment of a middle-aged man by the easy application of intermittent positive-pressure ventilation through a nose mask.
متن کاملCentral alveolar hypoventilation syndrome due to surgical resection for bulbar hemangioblastoma.
A 29-year-old man with a history of resected bulbar hemangioblastoma was admitted to hospital with nighttime breathing disturbance, but with apparently normal breathing while awake. After diagnostic work-up, including polysomnographic testing, he was diagnosed as having central alveolar hypoventilation syndrome due to surgical resection for bulbar hemangioblastoma. Non-invasive positive pressur...
